Virginia Medical Cannabis Certification Requirements

A State or Federal Issued ID

Proof of Virginia Residency (If you have a Virginia ID it will act as proof of residency)

A qualifying condition that you are seeking relief from.

Physical medical records are not required however can be requested by our practitioners under certain circumstances.

Physical Medical Records Documenting the Health Condition(s) for which you are being evaluated.
